New ACO Model Helps Pathways Health Partners Provide Better Care for Patients with Traditional Medicare

Improving the quality of healthcare has been a debate for quite some time now, but what seems to be completely bi-partisan and understood, is that everyone wants to create a system that helps to better meet the needs of all people. Focused on reaching that goal, the Centers for Medicare & Medicaid Services (CMS) announced a redesigned Accountable Care Organization (ACO) model that better reflects the agency’s vision of creating a health system that achieves equitable outcomes through high quality, affordable, person-centered care.

The new model is named the ACO Realizing Equity, Access, and Community Health or ACO REACH. It is a redesigned model of the Global and Professional Direct Contracting (GPDC) Model.

This new model heavily focuses resources on identifying and finding solutions to the social determinants of health. These health disparities include mortality, life expectancy, burden of disease, mental health, lack of access to care, etc.
